Does Humana Cover Drug & Alcohol Rehab?

What Humana Medicare Advantage, Medicare Supplement, employer and TRICARE coverage typically mean for addiction treatment, explained honestly.

Humana is a familiar name, but the coverage behind it has changed a lot in recent years. Today, most people who carry a Humana card have a Medicare-based plan, such as Medicare Advantage or a Medicare Supplement. Humana announced in 2023 that it would leave the employer group medical business, and it stopped selling individual marketplace plans years earlier. A smaller number of people still have Humana employer coverage or a Humana Military card for TRICARE.

That matters for addiction treatment, because Medicare-based coverage follows different rules from the employer PPO plans most private treatment programs work with. This page explains those differences honestly, so you know what to ask before you call. For the general basics, see our insurance overview.

Quick answer

Humana plans include substance use disorder benefits, but most Humana members today have Medicare-based coverage, which has its own rules about which providers and levels of care it pays for. Whether treatment at Reggie's can be covered at least in part depends on your exact plan. Reggie's is a DHCS-licensed, Joint Commission–accredited program in Los Angeles that primarily works with PPO plans; call (818) 355-1200 for a free, honest check.

Does Humana cover drug and alcohol rehab?

Humana plans include benefits for substance use disorder treatment, as federal rules require for most health coverage. Medicare covers treatment for alcohol and drug use disorders when it's medically necessary, and Medicare Advantage plans must cover at least what Original Medicare covers. Federal parity protections also apply to many plans, limiting how much more restrictive addiction benefits can be than medical ones.

Many Humana members today have Medicare-based coverage, which shapes where and how care is covered.

The harder question is where and at what level you can use those benefits. With Medicare-based coverage, that depends on the plan's network, whether a provider participates in Medicare, and what kind of setting the care is delivered in. The answer varies far more from plan to plan than most people expect, which is why we check each one individually.

Which kind of Humana coverage do you have?

Start with the card. The plan name and the words printed on it usually tell you which set of rules applies.

How do Medicare-based plans cover substance use treatment?

According to Medicare.gov, Medicare covers substance use disorder care across several parts. In broad terms:

  • Part A covers inpatient care in a hospital, including medically necessary inpatient detox in a hospital setting.
  • Part B covers outpatient services, including counseling, partial hospitalization and, since 2024, intensive outpatient program services from eligible providers.
  • Part D covers prescription drugs, which can include medications used to treat opioid and alcohol use disorders.

What Original Medicare generally doesn't include is a benefit for longer-term residential rehab outside a hospital setting. Some Medicare Advantage plans handle this differently, and some don't. That gap is one of the most important things to ask about if you're considering residential treatment.

Good to know: California's Health Insurance Counseling and Advocacy Program (HICAP) offers free, unbiased Medicare counseling. A HICAP counselor can help you compare what your Humana plan covers, separate from any treatment provider.

Can you use a Humana Medicare plan at a private rehab?

Sometimes, but it depends on details we have to confirm. Medicare-based plans can generally pay only providers that are eligible to bill Medicare for that service, and Medicare Advantage HMOs usually require you to use their network outside of emergencies. Medicare Advantage PPOs are more flexible, though out-of-network cost sharing is usually higher.

We primarily work with PPO insurance plans, and Medicare-based coverage is a different system. Rather than guess, we'll check your specific plan when you call and tell you plainly whether it can be used for the level of care you need. If it can't, we'll help you think through other options, including resources your plan does cover.

One call to admissions can tell you how your Humana plan treats each level of care before you decide anything.

What if you have a Humana employer plan or TRICARE?

If you still have Humana coverage through an employer, confirm it's active first, since many employer groups have moved to other carriers. If it is, it generally works like other commercial coverage: PPO-style plans often include out-of-network benefits, while HMO and EPO plans usually stay in network except emergencies. Whether the plan is self-funded by your employer or fully insured also affects which rules and regulators apply.

A Humana Military card means TRICARE coverage. TRICARE covers substance use disorder treatment under its own rules for authorization, provider certification and cost sharing, which differ from both Medicare and commercial plans. TRICARE's website explains those rules, and we can tell you what we're able to confirm.

Does Humana require prior authorization for rehab?

Insurers, including Medicare Advantage plans, commonly require prior authorization for inpatient detox and inpatient or residential care, then use ongoing utilization review to decide whether to approve more days. Outpatient care such as PHP or IOP may need authorization too. Medicare Supplement plans follow Original Medicare, which handles review differently. When care is urgent, such as withdrawal that's starting, tell the admissions team right away; urgent requests are generally processed faster.

If this is an emergency: severe alcohol or benzodiazepine withdrawal can be life-threatening. Call 911 or go to the nearest emergency room. Medicare-based and commercial plans cover emergency care regardless of network.

Is Reggie's in network with Humana?

Reggie's is an out-of-network provider for every insurance company, including Humana — we are not in-network with any plan. What we can do is check how your specific Humana plan treats our programs, explain what you'd likely owe, and say so plainly if your plan isn't a fit.

Costs on Medicare-based plans work a little differently. Medicare Advantage plans have an annual out-of-pocket limit for in-network covered care, and PPO versions usually have a higher limit that includes out-of-network care. Copays may apply per day of inpatient care or per outpatient visit. A Medicare Supplement may cover much of Original Medicare's cost sharing, depending on the plan letter. We walk you through every number before you commit to anything.

What should you have ready to verify Humana benefits?

  • Your Humana card, front and back, and your red, white and blue Medicare card if you have Original Medicare
  • The plan name exactly as printed (for example, the HMO or PPO version)
  • Member ID, and the name and date of birth of the person seeking care
  • Any Part D or other drug coverage card, if separate
  • Any second insurance, such as a spouse's employer plan

If you have more than one plan, coordination of benefits rules decide which pays first, and that can change the picture significantly. Our free Insurance Decoder can help you read your plan documents, and our admissions team is available 24/7.

What if Humana denies coverage?

You have appeal rights on every kind of plan, but the path depends on the coverage. Medicare Advantage and Medicare plans follow Medicare's appeals process, which starts with a request for reconsideration by the plan and can move to an independent reviewer; your denial notice explains the steps and deadlines. Expedited appeals are available when waiting could seriously harm your health.

For a Humana employer plan, the usual route is an internal appeal followed by external review. Which regulator can help depends on your plan: in California, the Department of Managed Health Care (DMHC) Help Center, 1-888-466-2219, oversees most HMO and many PPO plans, and the California Department of Insurance, 1-800-927-4357, oversees others. Self-funded employer plans fall under the U.S. Department of Labor. TRICARE has its own appeals process. Keep copies of every letter and note every call.

FAQs

Humana Rehab Questions

Humana Medicare Advantage plans must cover at least what Original Medicare covers for substance use disorder care, including medically necessary inpatient and outpatient treatment. Whether that applies at a specific private program depends on the plan's network, whether it's an HMO or PPO, and whether the provider can be paid under Medicare. We'll check your plan for free and tell you plainly.

Original Medicare covers inpatient care in a hospital and outpatient services such as partial hospitalization and intensive outpatient programs from eligible providers. It generally doesn't include a benefit for longer-term residential rehab outside a hospital setting. Some Medicare Advantage plans differ, so it's worth asking your plan directly or having us check.

Humana announced in 2023 that it would exit the employer group medical business, so many employer groups have moved to other carriers. If you still have a Humana employer card, confirm with your HR department or Humana that it's active. If it is, it generally works like other commercial coverage.

Medications used to treat opioid use disorder, such as buprenorphine (Suboxone), are often covered under a plan's drug benefit, including Medicare Part D, subject to the plan's formulary rules. Prescriber visits are billed separately. At Reggie's, medication-assisted treatment can include buprenorphine and naltrexone when clinically appropriate.

A Medicare Supplement doesn't decide coverage on its own. Original Medicare decides what's covered, and the supplement helps pay Medicare's cost sharing, depending on the plan letter. If Medicare doesn't cover a service, a supplement generally won't either. California's HICAP program offers free counseling if you'd like a second opinion on your coverage.
