Does Cigna Cover Rehab?
How Cigna employer plans typically cover drug and alcohol treatment in California, and how to check your own benefits for free.
Most people with Cigna in California are covered through work, often by a large employer, and that shapes almost everything about how rehab benefits work. Two coworkers can both carry a Cigna card and have very different addiction treatment benefits, because the employer, not just the carrier, designs the plan.
This page explains how Cigna plans typically cover drug and alcohol treatment, what the network name on your card tells you, how Cigna's behavioral health side works, how an employee assistance program fits in, and how Reggie's Recovery Center verifies your benefits for free.
Quick answer
Most Cigna plans cover drug and alcohol rehab at least in part, depending on your plan, because substance use treatment is an essential health benefit and federal parity law applies. Reggie's primarily works with PPO-style plans, and many Cigna PPO and Open Access Plus plans include out-of-network benefits. Our DHCS-licensed, Joint Commission–accredited program offers detox and residential care in Sun Valley. Call (818) 355-1200, 24/7, for free verification.

Does Cigna cover drug and alcohol rehab?
In most cases, yes, at least in part. The Affordable Care Act makes substance use disorder services an essential health benefit for individual and small-group plans, and the federal parity law requires plans that cover these services, including large employer plans, to apply limits no stricter than those for comparable medical care. California's SB 855 adds protections for state-regulated plans.
None of these rules guarantee full payment at a particular program. Cigna still reviews medical necessity, and your plan's network rules and cost sharing still apply.
What does the network name on your Cigna card mean?
Cigna cards usually print a network name on the front. That name is the quickest way to tell whether out-of-network treatment might be covered. These are general patterns; your plan documents are the authority.
| Name on your card | How it usually works | Out-of-network rehab |
|---|---|---|
| Open Access Plus (OAP) | No referrals required; broad national network | Often included, depending on what the employer chose |
| Open Access Plus In-Network, or "IN" plans | Same network, but coverage generally limited to it | Usually not covered except emergencies |
| PPO | In- and out-of-network care allowed | Often covered in part, at a higher cost share |
| LocalPlus | A smaller, local network, often in-network only | Usually not covered except emergencies |
| HMO or Network POS | Care coordinated through a primary care physician | HMO: usually not; POS: sometimes, with referral rules |
"Open Access" means you can see specialists without a referral. It doesn't, by itself, mean out-of-network benefits. The same Open Access Plus network can come with or without them, which is why we check the plan, not just the logo.
Why does your employer's plan design matter so much?
Cigna administers a large number of self-funded employer plans. In a self-funded plan, your employer pays the claims and hires Cigna to run the network, process claims and handle authorizations. Your card says Cigna, but the employer decides the deductibles, out-of-network benefits and many coverage details, within federal rules. Those plans are governed by federal law (ERISA) rather than California's insurance regulators.
A fully insured Cigna plan, more common with smaller employers, is regulated by the state. In California, Cigna's HMO plans fall under the Department of Managed Health Care, and its insured policies generally fall under the Department of Insurance. Your HR or benefits team can tell you which you have. Cigna also administers international and expatriate plans; those follow their own policy terms, and we verify them the same way.
Who handles behavioral health on Cigna plans?
On many Cigna plans, mental health and substance use benefits are managed by Evernorth Behavioral Health, part of The Cigna Group (formerly known as Cigna Behavioral Health). In practice, that means the people who authorize detox or residential care may sit in a different department from the ones who handle your medical claims. Look on the back of your card for a behavioral health or mental health and substance use number; that's the right line for rehab questions.
The myCigna website and app also show your plan documents, benefits and Explanation of Benefits statements. Some employers carve behavioral health out to a different company entirely; if so, its name and number will appear on the card or in your benefits guide.
Is an employee assistance program the same as rehab coverage?
No, though the two are easy to confuse. Many employers offer an employee assistance program (EAP), sometimes administered by the same company as their health plan. An EAP typically provides a small number of free, confidential counseling sessions and referrals. Addiction treatment itself is paid through your medical plan's behavioral health benefits.
Employee assistance program
- A few short-term counseling sessions, usually at no cost
- Help with referrals and next steps
- Not designed for detox or residential care
- Confidential; your manager isn't told
Medical plan behavioral health benefits
- Covers detox, residential, PHP and IOP when medically necessary
- Deductible, coinsurance and network rules apply
- Pre-authorization commonly required for inpatient care
- What we verify when you call
Which levels of care are commonly covered?
Insurers including Cigna commonly evaluate each level of care separately, often using criteria from the American Society of Addiction Medicine (ASAM). Generally: inpatient medical detox (typically 5 to 10 days at Reggie's) is commonly covered when withdrawal risk is documented. Residential treatment (30 to 90 days, by clinical need) is often covered when your plan's benefits apply to the program, approved in stages. PHP and IOP at our Los Angeles outpatient center are frequently covered, often with different cost sharing. Medication-assisted treatment visits are often covered, with medications sometimes under the pharmacy benefit. Sober living is usually not covered.
Does Cigna require pre-authorization for rehab?
For inpatient detox and residential treatment, plans including Cigna commonly require pre-authorization, followed by ongoing utilization review. The provider submits an assessment and history, the plan approves an initial number of days, and the clinical team sends updates to request more while care remains medically necessary. Our admissions team submits authorizations for you, and our clinicians handle the reviews.
Is Reggie's in or out of network with Cigna?
It depends on your plan and network, so we confirm it during verification rather than claim it in general. If your Cigna plan includes out-of-network benefits, Cigna typically pays part of covered care based on its allowed amount, usually with higher coinsurance and sometimes a separate deductible and out-of-pocket maximum. If it's an in-network-only plan, out-of-network care is generally not covered outside emergencies, and we'll say so plainly and help you look at other options, including self-pay.
What should you ask about your Cigna plan?
We ask these on every verification call, and you're welcome to ask them yourself:
- Does my plan have out-of-network behavioral health benefits, and what's the separate deductible?
- What coinsurance applies to inpatient detox, residential, PHP and IOP?
- How much of my deductible and out-of-pocket maximum have I met this year?
- Is pre-authorization required, and who handles behavioral health authorizations?
- Is my plan self-funded or fully insured?
Our free Insurance Decoder can translate your Cigna SBC first. To verify, call (818) 355-1200 or use our insurance page form with your card (front and back) and the patient's and policyholder's names and dates of birth.
What if Cigna denies coverage?
Read the denial letter for the reason, criteria and deadline, and ask whether your clinician can request a peer-to-peer review. Then file an internal appeal with a letter of medical necessity; if it's upheld, you can usually request an independent external review. For a self-funded plan, federal rules apply and the U.S. Department of Labor can help. For state-regulated plans, which regulator depends on your plan: the California DMHC Help Center (1-888-466-2219) for most HMO and many PPO plans, or the Department of Insurance (1-800-927-4357) for others. Our insurance guide covers each step.

Cigna Rehab Coverage Questions
Many Cigna plans cover inpatient detox and residential treatment, at least in part, when medically necessary. Coverage usually requires pre-authorization and is approved in stages with ongoing reviews. What's paid at a particular facility depends on your network, employer plan design, deductible and coinsurance. A free verification at (818) 355-1200 shows your actual benefits.
No. Reggie's is an out-of-network provider for every insurance company, including Cigna — we are not in-network with any plan. Reggie's primarily works with PPO-style plans, and many Cigna PPO and Open Access Plus plans include out-of-network benefits. We'll explain how yours applies before you decide anything.
Sometimes. Open Access Plus means no referrals are needed, but whether out-of-network care is covered depends on how your employer set up the plan. Some OAP plans include out-of-network benefits with a separate deductible; "In-Network" versions generally don't. Verification tells you which one you have.
Evernorth Behavioral Health is part of The Cigna Group and manages mental health and substance use benefits for many Cigna plans. It was formerly known as Cigna Behavioral Health. If your card lists a behavioral health number, that's typically the line for rehab benefits and authorizations.
No. Verifying benefits doesn't notify your employer, even on a self-funded plan, and substance use treatment records have extra federal protection under 42 CFR Part 2. If you need time off, our team can help with FMLA paperwork. If the plan is a spouse's or parent's, many plans let you request confidential communications.
An employee assistance program usually offers a few free counseling sessions and referrals, not detox or residential care. It can be a helpful first step, but treatment itself is paid through your medical plan's behavioral health benefits. We can verify those benefits for free, usually in 15 to 30 minutes.
