Does Anthem Blue Cross Cover Rehab?
How Anthem Blue Cross plans, and out-of-state Blue cards, typically cover drug and alcohol treatment in California.
Anthem Blue Cross is one of the most common names on insurance cards in Los Angeles, and one of the most commonly confused. It isn't the same company as Blue Shield of California, and a card with a Blue Cross logo from another state works differently again. Getting those details right is the first step to understanding your rehab benefits.
This page explains how Anthem Blue Cross plans typically cover drug and alcohol treatment, how to tell which kind of Anthem plan you have, how out-of-state Blue cards work in California, and how Reggie's Recovery Center verifies your benefits for free.
Quick answer
Most Anthem Blue Cross plans cover drug and alcohol rehab at least in part, depending on your plan, because substance use treatment is an essential health benefit protected by federal parity law. Reggie's primarily works with PPO plans, and many Anthem PPO plans include out-of-network benefits. We're DHCS-licensed and Joint Commission–accredited, with detox and residential care in Sun Valley. Does Anthem Blue Cross cover drug and alcohol rehab?
Generally, yes, at least in part. The Affordable Care Act lists substance use disorder services as an essential health benefit for individual and small-group plans, and the federal parity law requires plans that cover these services to apply limits no stricter than those for comparable medical care. In California, SB 855 also requires state-regulated plans to cover medically necessary substance use treatment using generally accepted standards of care.
Those protections shape what's covered, not how much you'll pay at a particular facility. That still depends on your plan type, network rules and cost sharing, which is why the details below matter.
Is Anthem Blue Cross the same as Blue Shield of California?
No. California is one of the few states where the Blue Cross and Blue Shield names belong to two separate companies. Anthem Blue Cross is the trade name of Blue Cross of California, an independent licensee of the Blue Cross Blue Shield Association and part of Elevance Health (the company formerly called Anthem, Inc.). Blue Shield of California is a different, nonprofit company with its own plans, networks and phone numbers.
Which Anthem Blue Cross plan do you have?
Anthem sells employer group plans, individual plans (including through Covered California in many regions), and Medicare Advantage plans. The network type printed on your card is the single most useful clue to how rehab benefits will work.
| Plan type | How it usually works | Out-of-network rehab |
|---|---|---|
| PPO | Broad network, no referrals, out-of-network care allowed | Often covered in part, at a higher cost share |
| EPO | No referrals, but coverage generally limited to the network | Usually not covered except emergencies |
| HMO | Care through a primary care physician and medical group | Usually not covered except emergencies |
| Medicare Advantage | Medicare benefits through Anthem, under federal rules | Varies; some PPO versions include it |
| Medi-Cal | California's Medicaid program | Specialty addiction treatment is generally arranged through county behavioral health systems |
Which regulator oversees your Anthem plan?
Anthem has two kinds of California plans. Blue Cross of California is a health care service plan overseen by the Department of Managed Health Care (DMHC); this covers its HMOs and many of its PPO and EPO plans. Anthem Blue Cross Life and Health Insurance Company issues insurance policies overseen by the Department of Insurance (CDI). The legal name on your plan documents tells you which applies. If your employer is self-funded and only uses Anthem to administer the plan, federal law governs it instead.
What if your Blue Cross or Blue Shield card is from another state?
Many people who come to Los Angeles for treatment carry a Blue plan from elsewhere, such as an employer plan from Texas or Illinois. Out-of-state Blue Cross and Blue Shield plans generally use the BlueCard program, which lets members access care through the local Blue plan's network while their home plan sets the benefits and authorization rules.
- A suitcase logo on the card usually signals BlueCard eligibility; "PPO" inside the suitcase signals a PPO-type plan.
- The first three characters of your member ID (the prefix) identify your home plan. We need the whole ID, prefix included.
- Your home plan's benefits, not California's local plan, decide what's covered and whether pre-authorization is needed.
Federal employees in the Blue Cross Blue Shield Service Benefit Plan (often called FEP) follow that program's own benefit rules and federal appeal process. We verify any of these Blue plans the same way.
What levels of care do Anthem plans commonly cover?
Insurers commonly assess each level of care separately, often using criteria from the American Society of Addiction Medicine (ASAM). This is how coverage typically lines up with the care Reggie's provides.
| Level of care | How coverage commonly works |
|---|---|
| Inpatient medical detox (typically 5 to 10 days) | Commonly covered when withdrawal risk is documented; pre-authorization usually required |
| Residential treatment (30 to 90 days, by clinical need) | Often covered on PPO plans; approved in stages with ongoing review |
| Partial hospitalization (PHP) | Frequently covered as a step down from residential |
| Intensive outpatient (IOP) | Widely covered; per-day or per-session cost sharing may apply |
| Dual-diagnosis care | Mental health treatment alongside addiction care falls under the same behavioral health benefits |
| Sober living | Usually not covered; outpatient care attended while living there often is |
How do pre-authorization and reviews work with Anthem?
For inpatient detox and residential care, insurers including Anthem commonly require pre-authorization before admission or within a short window after an urgent one. The treatment provider submits an assessment and history, and the plan approves an initial number of days. After that, the clinical team sends regular updates (utilization review) to request more days while the level of care remains medically necessary.
We submit authorizations for you and our clinicians handle the reviews. If you carry an out-of-state Blue card, the authorization request generally goes to your home plan, so expect us to ask for the full member ID.
Is Reggie's in or out of network for your Anthem plan?
We don't make blanket network claims, because status varies by plan and network and can change. Verification tells you exactly where your plan stands. In general terms: if your Anthem plan is a PPO with out-of-network benefits, Anthem typically pays a portion of covered care based on its allowed amount, with a higher share for you and sometimes a separate deductible. If it's an HMO or EPO, out-of-network care is usually not covered outside emergencies, and we'll be upfront about that and help you weigh other options, including self-pay.
What will you owe with an Anthem plan?
Your share depends on your deductible, your coinsurance after the deductible, and your out-of-pocket maximum. Out-of-network care on Anthem PPO plans often has its own deductible and maximum. We don't quote prices here; we walk you through your actual numbers in plain language before you commit to anything. Our free Insurance Decoder can also translate your Summary of Benefits and Coverage before you call.
Where is the behavioral health number on your Anthem card?
Check the back of the card. Many Anthem cards list a behavioral health or mental health and substance use number alongside the general member services number; that's the right line for rehab questions. Anthem members can also sign in at anthem.com or use the Sydney Health app to view plan documents, benefits and claims. Some employer plans route behavioral health benefits through a separate company; if so, its number will be on the card.
To verify, have your card (front and back) and the name and date of birth of the person seeking treatment and of the policyholder, if different. Call (818) 355-1200 or use the form on our insurance page; answers usually come within 15 to 30 minutes, and the admissions process can often begin the same day.
What if Anthem denies coverage?
Start with the denial letter: it should state the reason, the criteria used and your deadline. Your clinician can often request a peer-to-peer review with Anthem's reviewer. If that doesn't resolve it, file an internal appeal with a letter of medical necessity; if it's upheld, you can usually request an independent external review.
Which regulator handles it depends on your plan. For most HMO and many PPO plans, contact the California DMHC Help Center (1-888-466-2219). Anthem Blue Cross Life and Health policies go to the California Department of Insurance (1-800-927-4357). Self-funded and out-of-state plans follow their own rules. Our insurance guide covers appeals step by step.

Anthem Blue Cross Rehab Questions
Many Anthem plans cover residential treatment, at least in part, when it's medically necessary. Coverage is usually approved in stages after pre-authorization, followed by ongoing clinical reviews. How much is paid at a specific facility depends on your plan type, network rules, deductible and coinsurance. A free verification shows what your plan includes.
No. Reggie's is an out-of-network provider for every insurance company, including Anthem Blue Cross — we are not in-network with any plan. Reggie's primarily works with PPO plans, and many Anthem PPO plans include out-of-network benefits. We'll explain how your plan applies before you decide anything.
Often, yes. Out-of-state Blue plans generally use the BlueCard program, which gives members access to care in other states while the home plan sets benefits and authorization rules. Look for a suitcase logo on your card, and give us the full member ID, including the three-character prefix, so we can verify.
Medical detox is commonly covered when there's documented withdrawal risk, such as with alcohol, benzodiazepines or opioids. Pre-authorization is usually required for inpatient detox. Our team submits the clinical information on your behalf. If withdrawal has started, tell us right away so the request can be treated as urgent.
HMO plans usually cover care only within their network and medical groups, except emergencies, so coverage for our program is more limited. Call us anyway: we'll check your plan honestly, explain any options, and point you toward resources if our program isn't the right financial fit. Self-pay and financing can also be discussed.
In California, specialty substance use treatment for Medi-Cal members is generally arranged through county behavioral health systems and their contracted providers, rather than through the Medi-Cal health plan. We generally don't accept Medi-Cal, but we'll connect you with a provider or county program that does. The SAMHSA National Helpline (1-800-662-4357) can also help.
